Only complete this form if you have an eligible Pensioner Concession Card or Veteran Gold Card and you live in the property you're claiming the pension rebate for. 

If you have a Veteran Gold Card, it must be embossed with one of these: 

  • WAR WIDOW or WIDOWER 
  • EDA (Extreme Disablement Adjustment) 
  • TTI (Totally and Temporarily Incapacitated) 
  • TPI (Totally and Permanently Incapacitated).

  • Death certificate if the owner is deceased and there’s no will – you’ll also need to attach a letter from a solicitor confirming there’s no will

        The property owner must be deceased and you must be nominated in their will as the beneficiary.

            To support this application, you must provide a deed of agreement signed by a solicitor. The agreement should contain details as in this sample deed of agreement.

                    • This consent remains valid while you are a customer of Sydney Water unless you withdraw it by contacting Sydney Water or Services Australia.
                    • If you withdraw your consent, you will not be eligible for Sydney Water's pension rebate.
